循環器の分野では心臓カテーテルについても同様の構造がある。日本の循環器内科医は優秀である。急性心筋梗塞がくると、夜中だろうがなんだろうがさっと集 まり匠の技で詰まった血管を通していく。ところが原材料費が異常に高くカテーテル代とステント代で200万円程度かかることもざらである。夜間に何人も集 まって生命を救ったことに対する技術料(手術料)はたったの22万円程度のもので、これとて医師がもらえるわけではない。1回呼ばれると5000円程度の 報酬であるところはざらである。

医療側としては、これだけ使っているのだから市場原理にのっとって安く輸入して、きちんと技術料を払ってほしいが、厚労省はこの部分でタフに交渉をしてく れる気はないらしい。高いデバイスを使わせて、人件費を抑えて帳尻を合わせている。高額な医療費はほとんど病院を素通りして企業に流れていく。医師には働 いた分の報酬さえきちんと支払われていない。原価計算をしていない保険点数を責めるべきか、営利企業を責めるべきか。いずれにせよこれを世に問う術がな い。
医療は公定価格と営利目的の企業の論理の狭間に落ちたままの業界である。
